As a member of our American Health Data Institute (AHDI) team, you will use your education and clinical background to promote wellness and prevent complications in our chronically ill member population. In the rewarding role of a Health Care Navigator Nurse you will use both your clinical and interpersonal skills on a daily basis, reaching out to the chronically ill and educating them to improve their self management of their chronic conditions.
This Monday through Friday, part-time, afternoon/evening position offers the satisfaction of being able to use your knowledge and experience to lead our members to better health, while giving you an alternative to the clinical environment.

The qualified candidate will have:
- A current, unrestricted RN license in the state of Indiana
- A strong, clinical background -preferably 3-5 years in acute care or skilled or rehabilitative clinical settings
- The ability to work independently and as part of a team
- The ability to be flexible and to multi-task
- Strong written and oral communication skills
- Excellent data entry and computer skills
Bi-lingual (Spanish) is a plus, but not required.
A background in Case Management and/or Precertification is not required, but would be a definite plus.
